Financing Options
The cost of surgery or non-surgical procedures are comprised of the surgeon’s fee, anesthesia fees, laboratory fees, supplies, implants, and products. (Patient dependent). The Center for Cosmetic Surgery accepts all major credit cards, (including Apple Pay, Google Pay, Android Pay), CareCredit, and checks or cash.
Apply online for other services such as credit cards or lenders such as Prosper Healthcare Lending, SoFi, LightStream, or LendingPoint.

Forms can be filled out in your browser via Adobe Reader or Acrobat, depending on browser type. Note that Adobe Reader cannot save typed data so we suggest printing copies for your records.
The New Patient Form is basic background health information. It will be reviewed by your doctor and become a confidential part of your medical record.
New patients can save time during their first appointment by completing the forms prior to their visit. Please remember to bring all completed forms with you to your appointment.
